Hi Jorge,
You've successfully scheduled the following Quest
Diagnostics appointment. To help you have the best experience
possible, please arrive and check in 10 minutes before
your appointment time. If you check in more than 10 minutes
late for your appointment, you will be served as a
walk-in.
When testing is billed to your health plan, Quest
has an easy way for you to arrange for payment for your portion of
lab testing for your visit, if any. Simply present your
major credit card, debit card, or your health savings card
during your visit. Please make sure to pay
any overdue balances before you arrive.
Below you'll find a code to speed up your check-in process. Scan your code at the kiosk and follow the on-screen
instructions.

If your test requires it, don't forget to
fast for 8-12 hours before your appointment. If
you're not sure, contact your doctor's office. Learn more

Make sure to bring your
insurance card, a valid photo ID for proof of identity
and a credit, debit or health savings
account card.

|
You will need to have your
physician or employer-sponsored test order (electronic or paper)
or have purchased a test online from Quest.
If your doctor or employer
sent an electronic order to Quest, or your purchased a test
online with Quest, we will be able to access your order.
If you have a paper order, you will need to provide it
at your appointment.

Self-pay good faith price estimate

If you will be paying out-of-pocket for your lab test(s) instead of using insurance, Quest
is providing you a price estimate. This estimate (also called a good
faith estimate) is available to self-pay customers in select states.
