Good afternoon Jorge,
We are thrilled to welcome you to Boca Fertility! It was a pleasure speaking with you and we are happy to assist you with your fertility journey. As per Dr. Peress and myself, there is some testing we recommend for you. I have attached everything you will need - please print and review. I have listed everything below as well. Please contact the office, our IVF team, Amy, Mariah or myself if you have any questions or concerns.
1) FDA infection screening bloodwork – Please print and complete the attached infection screening bloodwork at Quest at your earliest convenience. No fasting required.
2) Sema4 – Genetic blood test that is done at our office, any day of the week. Please call our office to schedule. The cost for the blood draw is $40 – we will supply the laboratory with your insurance information. If insurance does not pay for the testing the cost is $249 at most. Results take up to 21 business days, if you are found to be a carrier for anything, a genetic counselor will contact you with their recommendations.
3) Psychological evaluation – If not already completed, this is required for you to complete prior to proceeding with donor egg IVF and surrogacy. Please schedule a consultation at your earliest convenience (can be virtual). Below I have listed providers that we highly recommend:

4) Semen culture – The culture requires collection at home with one of our sterile containers, by appointment only. Please call to schedule drop-off at our office Monday – Friday 9am-2pm. Collection must be by masturbation, and only mineral oil can be used as a lubricant. Collection must occur within 1-2 hours of drop off time. No abstinence required. I have confirmed with embryology that the analysis is sufficient to proceed with IVF. No need to follow up with urology as that will delay the process.
5) Egg donor selection – Once you have confirmed with the egg donor agency that you are proceeding with the selected egg donor, they will contact us to start the process.
I have attached pricing for donor egg IVF and gestational surrogacy for your review. Additional fees as listed at the bottom of the donor egg IVF fee sheet include IVF medications ($5,000-$6,000).
Once the transfer is
complete the remaining embryos are stored at Reprotech. To find out
